I am a Neurology Inpatient Care Coordinator that specializes in patient care prior to discharging from the hospital. Responsibilities are handling medical records requests for continuation of care, scheduling outpatient appointments and ordering durable medical equipment, in addition to working with the rest of the inpatient care management team.
